Lichen sclerosus et atrophicus (LSA) or, simply, lichen sclerosus, is a disorder characterized by an atrophic epidermis, altered collagen in the superficial dermis, and an overlying white or white-blue appearance to the skin.
Lichen sclerosus is a dermatosis that most commonly occurs in the anogenital region in females and in the genital region in males. Clinically, lichen sclerosus is characterized by erythematous macules, papules, and plaques that resolve as an atrophic patch with a characteristic “cigarette paper” appearance.96 Lichen sclerosus shows a marked female predominance, and extragenital lesions can be seen in the trunk, neck, and upper and lower extremities. Lesions involving the penis are desig nated balanitis xerotica obliterans, a common cause of phimosis. An important com plication of lichen sclerosus in females with genital involvement is the development of dysplasia and squamous cell carcinoma; this phenomenon has not been described in association with extragenital lesions. The cause of lichen sclerosus is unknown, but there is an association between this entity and the presence of class II antigens (HLA-DQ7, DR7, DQ8, and DQ9).97 Also, a moderate number of patients with lichen sclerosus have other associated autoimmune diseases, including hypothyroidism, diabetes, pernicious anemia, and vitiligo.
